Degenerative disease of synovial joints that causes progressive loss of articular cartilage

knee OA (symptomatic) more than hip OA

Females > males

African American males are the least likely to receive total joint replacement when compared to whites and Hispanics

Pathoanatomy: eventual decrease in amount of proteoglycans

Inheritance: non-mendilian

PE:
Range of motion in OA:
lack of full extension (>5 degrees flexion contracture)
lack of full flexion (flexion <110 degrees)

Xray findings:
joint space narrowing
osteophytes
eburnation of bone
subchondral sclerosis
subchondral cysts

Treatment:

Non-op:
1) non-steroidal anti-inflammatory drugs
2) rehabilitation, education and wellness activity
3) weight loss programs if symptomatic arthritis and BMI > 25

Op:
1) high-tibial osteotomy - younger patients with medial unicompartmental OA
2) unicompartmental arthroplasty (knee) - isolated unicompartmental disease
3) total knee arthroplasty
